Manual meal-logging creates significant friction for patients enrolled in provider-run chronic disease programs, who must type or navigate menus to record dietary intake, leading to high abandonment rates and loss of clinical visibility into dietary adherence. Voice-based meal logging is the lowest-friction modality, but it processes Protected Health Information (PHI) on behalf of provider customers acting as HIPAA covered entities and requires end-to-end HIPAA Security Rule compliance with no room for post-deployment remediation. Without continuous, accurate nutrition data, clinicians cannot intervene when patients deviate from treatment plans, directly impacting clinical outcomes, and the platform needed to launch within a fixed 16-week timeline.
Aivar designed and implemented a serverless, API-first microservice architecture focused on Voice Intelligence, HIPAA-Aligned Security, and Cost-Optimized AI Inference.
Aivar conducted a Total Cost of Ownership analysis to validate the cost-optimized AI inference strategy: the serverless Lambda architecture eliminated fixed compute capacity in favor of pay-per-invocation pricing, and the two-model Bedrock split — Claude 3.5 Haiku for lightweight food extraction and Claude Sonnet reserved for higher-value nutrition analysis — matched inference spend to task complexity rather than routing all requests through a single premium model.
